If it would help, I could add a second page to the Flex Tech order form with fill-in spaces for the individual purchaser’s name, mailing address, email address, phone number, credit card number, expiration date and 3 digit security code or any other information that you or Flex Tech think needs to be included…

Hi Randy,

I added a second page to the Flex Tech order form with fill-in spaces for the WOG Member's name, mailing address, email address, phone number, credit card number, expiration date and 3 digit security code, etc.
