Unfortunately I cannot answer how mine was lost but assumed it did not have a chain and it was set on top of the pump and left there, I will never know.
Here is what I can add though, I was going to add a chain to my new fuel cap but my brother carved a bird logo on it so I did not want to mess that up. So here is what I do, I found the cap hangs perfectly on the back of the fuel door from the key latch. I put it on the key latch, you cannot close the fuel door with it on there so it reminds you to put it back on, and I need all the help I can get!

My chain snapped. I haven't left my fuel cap anywhere yet, but I figured i might as well get a spare for when I do 😂

When I had my fuel tank out last year I found that my Spring wasn't so springy. It took me a couple of tries to get it squeezed enough to fit inside the opening but yet still catch the lip that holds it in. I could see how one would pull all the way out from the spring breaking and then getting left behind at a future fill up.
The chain on mine was also quite corroded. Seems a brass chain would have been a better choice.

Oh **** no. I've worked in Automotive my whole professional career, and now on the OE side, and I get to deal with engineers on the regular. One shop I worked in, this was the only tape in the player, and when it would happen, the boss would push the 8-track in and turn it up!

In my case the PO claims it came to him minus a cap. This coach was a rental on a campground/marina at $165/mo. for 3 years. Who knows what clientele like that did to this poor thing.
